Create for to add and edit data to JS file

I have a phone app that gets its information from a JS file. I'd like you to create a form that will add and edit data in the JS file so I don't have to do it manually.

The JS file is attached. There are 3 types (var) of information in the file: 1) trivia 2) iq 3) audio

At first I should have a choice to edit or add to one of these 3. Here are the following variables that need to be added when creating a new entry (with notes and samples):
