A Registration Page for Wordpress ultisite

I need a registration page for a wordpress multisite installation. The registration page must enable the user to register a new site, choose a membership type (based on user roles) and choose a template. After registering/activation the user is taken to their subdomain to login (as you know this is all part of wordpress anyway).
Clarification:
I do not to be quoted on setting up the template or role setup as this is done through plugins. The site is also up and running, so I really need the registration page tweaked as described below.
What I need the customer to be able to choose a template on signup, and then for the user to be able to login to this site at the sub-domain level. I use gravity forms for user registration for different roles but am happy for the standard wordpress registration process to be used or for someone to quote on a custom solution. You will need to be clear on this.
I don't have a huge budget for this as I would think this would be a couple of hours work maximum for someone that knows wordpress multisite.
