School Lunch Count Web Programming

SCOPE: We are a school in need of a web based system for teachers to provide lunch counts daily. This count/details would be published to a summary report for the kitchen staff. The site would provide a place for teachers to enter a count (number of students needing a meal) and a place for our kitchen staff to get totals.
There are four forms I'd need: K-2; 3-6; 7-9; 10-12
SAMPLE: Up til now, we've been using this page for ALL secondary students (7-12)- [login to view URL] It was created by a teacher at the school. It has all the content that we need for secondary and does it's job. It reports back to here: [login to view URL]

DREAM:
I need four different forms for K-2; 3-6; 7-9; 10-12 lunch periods. They would all report back with totals. I'd like the form to look clean and be mobile friendly. IF it's possible to use it in wordpress, that would just be a bonus.
TOTALS
I need a report page that show totals for each group and grand totals for all groups.
CLEARING -- I'd like this report to AUTO CLEAR (no button) at 3pm each day. (if that can be done) It would also be nice to have a print feature/button on the reports page.
