Application/Script Development

I need to have an application or script (not sure which this would be) created. I know what I want the item to do but just not sure how it would get done.

Obviously, I am not well versed in the application creation process and not clear on how to relay this but here are the basics.

I am looking to develop an item that pops out and captures information similar to an aWeber optin box. It seems like a very simple script however I want to incorporate other items. If you are familiar with what I might be looking for, please get in touch.
