Membership ID Program

We are a not-for-profit organization (Senior Center). We are in the process of creating membership ID cards for our Seniors who come to our center daily. We have tried many programs to do so, and can not figure out access well enough to do what we want it to do. Here is what we need: A program that will be called "Glenridge ID Cards" The program needs to allow us to input the following into a database: -Members Name -Adress -Phone Number -Picture of Member -Emergency Contact Information, Medication ------------------------------------------- We would need the program to do the following task: -Allow us to search for a members name -Print "ALL members ID's" or Print selected members ID's -Input date that ID card was printed into Member's database file. I have attached an image of what the print out should look like of the ID Card. Please submit any programs that you may have done similar or non similar to this project so I can see if you are a good candidate for this project.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
