Search Form needed for PHP website

hi,

We need a mySQL database search form designed into our existing php website.

what needs to be done:

1) search feature on php website, to be put into our website on the same row as the menu options, but on the right hand side align, like shown at:

2) 'go button' graphic created for us, to work seamlessly with the colour of our website, using the example shown at:

[login to view URL]

3) Search Results Page

A php webpage to display the results of search form submit.

We will supply the winning bidder with our website files, to incorporate the search form.

We will also supply the winning bidder with admin control panel details to our server, to create and setup the mySQL database required to make our search engine work.

The winning bidder will need to upload and implement the finished search form and results page designs and files into our server, which we will supply the winning bidder FTP details for.

Our budget for this project is $40-$80, so please do not go putting in high bids.

Aswell as sending us the new finished designs in the requested formats described above, ALL project source and design files are to be delivered to us aswell.

Payment will be done upon full satisfactory completion of the project.
