Create basic system to send SMS (PHP/MySQL, Single Page)

We have a need for a very basic system that we can use to send verification codes to our clients from a web page.
We have had the UI designed and coded into HTML, and we've attached this to the project, so it is just making the system work.
What the system needs to do:
- We need to be able to select a client, which will show us all contacts under that account. When we click "send verification" it needs to send an email (we will provide SMTP credentials) to our email to SMS service <contactphone @ [login to view URL]> with a 6 digit, randomly generated code, and a message that we'll specify (Something like "Your IT verification code is 123456, please provide this to your representative when requested or call 09 123 4567 if you were not expecting this code").
- We need to display the last 4 codes sent, along with the date, time and contact on the web page, and all other codes will save to the database.
- It needs to have backend database to store customer IDs, customer names and customer phone numbers as well as recently sent codes
- We need to be able to add new customers from the system
- We need to be able to add new contacts from the system
- We need to be able to edit existing contacts from the system
We do not need the ability to delete customers or contacts from the web - any deletion will be done manually via PHPMyAdmin.
We will upload the HTML version to our web host and provide you a login to cPanel so you can work directly in that environment.
