Display WordPress Hierarchical Taxonomy As Directory Plugin

Create a WordPress plugin that displays terms from a specified hierarchical taxonomy as a directory similar to classic web directories (Yahoo! being the best-known, but now defunct, example. The best example still remaining is DMOZ - [login to view URL]).
For example, if DMOZ was a WordPress site, the root page would display the top-level terms in a selected taxonomy:
[login to view URL]
When one clicked on a term, one would be taken to that term page which would display a directory of the child terms similar to DMOZ:
[login to view URL]
And, as can be seen at the link above, it should also show the links (or, in our case, posts) that belong to a selected term (however, it should not display the posts inside of child terms).
One should be able to continue to drill down through multiple levels of hierarchical terms, ex.:
[login to view URL]
The winning plugin must:
Follow best practices for WP plugin development.
Include in-code documentation per WP standards.
Provide a shortcode that one can use to display the directory.
Create a sensible permalink structure similar to that expressed in the link above.
Provide sensible default styling for the output directory.
