We need a Form that will post to sql and allow upload of image that will be linked to the submission.
## Deliverables
The form details are as follows.
A user begins the process of filling in the form with basic information.
Real Name
Address1
Address2
City State Zip
Phone Number
DOB
Desired Username
they will click Next button. At that time the script should save the data to MySQL and auto assign a temp password to that users submission and email them their temp password with links to login. when user first logs in they will be required to change the password.
after next button the system should then show the user the form to allow them to upload multiple image files. the amount of files required and name of files should be customizable and auto renamed upon saving to server. we would like a true upload progress bar. If user decides not to complete form they can come back and complete the uploads of images. using user and pass.
All data and images should be accessible with a secure admin page with searches for name city state date etc etc. we would also like to see the list of all submission sorted by date 100 lines then next for additional submissions.
This form should be completed template based for easy edits and graphics.
