Add two email forms into Flash file

I have a Flash CS3 file and i need to add two simple forms to it.?

Users will fill in the forms and the data will get emailed to a specific email address.?

The forms are:

1) ? "Subscribe to Newsletter" form - will have the following fields:

? ? ? First Name, Last Name, Company, Postal Address, Phone No, Email

2) ? "Contact Us" form - will have the following fields:

? ? ? Name, Company, Phone Number, Email Address, ? Message ? ? ?

The server will be running a PHP language, so you can use PHP to mail the form if necessary.
