PHP Script to submit PDF file

I need a PHP code to take a PDF that I have created for a customer and when they submit it, the data they added to the PDF will be on an attached e-mail sent to me. The attachment will be the entire PDF with the data filled in. The PDF data filled in will be the same each time but? a seperate portion? will be different for each customer so I can not use a "generic" form to capture the data each time.

I need this done today and should be easy for a PHP expert.
