Troubleshooting PHP/Javascript Aweber/Lead Capture Integration

I have a functioning script that integrates Aweber and a lead capture page provided by the company. This allows me to capture the prospect's name and e-mail in my Aweber account, while simultaneously putting them into the company's autoresponder, while the prospect has only filled out one form. This script is coded in PHP and has some Javascript.

I need to have this code work for a different company now. I am unable to get it to work properly due to this company's slightly different form HTML.
