Asp page throwing sql code back

This is a pretty easy project. I have homegrown helpdesk solution that was built a couple years ago, after coming on I have made several dozen small changes. Right now I am getting 1 error when creating certain types of job requests. The job is being created but instead of completing the creation process, the software stops halfway and throws my code back. Should be an easy fix under an hour, just one page to edit. I will but a screen capture up of what is happening. [login to view URL] is what happens when a job is processed correctly, [login to view URL] is the current screen when processing a job, and [login to view URL] explains of the software works. The application is on my intranet you will have no access, so I have zipped up the application and the winning developer will receive the zip file. If this works out I have other jobs available.
