Black & White Fax Flyer

I am a pharmacist and I contract out my services to different pharmacies. I would like a 8 1/2 x 11 inch flyer designed that I can fax to pharmacies advertising my services. I would like the ad to have a New Year's or a Winter theme and advertise that this is a special sale on bookings received for January 19th through February 28th in that there will be no travel costs charged to the client. Something like "Winter Sale - Save Big in the New Year" . I have attached my company logo and would like the ad to also state that "JP Doolittle Enterprises, PC is a relief Pharmacist service based in Richmond, VA with over 10 years of providing relief pharmacist coverage. I would like my name "Paul Doolittle, RPh" to be in the ad along with my contact phone number (1-800-921-4763) and my website address ([login to view URL]). This needs to be a clear ad - no shading as it will be faxed over the phone lines and received in a black and white fax machine.
