Develop and Write a Recruitment Questionnaire for Hiring of Virtual Assistants

I'm hiring some virtual assistants for my internet marketing business and I already got many applications. It's not easy to select the right applicants. For selecting the right candidates I need a comprehensive Recruitment Questionnaire which includes also a description of the position, the tasks and responsibilities. I'm thinking about a 4 to 5 pages Word document, but I'm open for suggestions. I already have some descriptions and questions which I used in the past.
