Cricket Match Result form

I woul like a cricket match result form that could be placed on our website for inputting league match results details. The form needs to get information on clubs, players from a database back end and when it is submitted I would like the data eamiled to me in csv format so i can import it into a db to produce league tables verages etc... I have attcahed an image of our current paperbased form.I have aslo attached an exsample db that should give an idea of the format for the backend. This would probably be a three part form. Page one Fixutre details and Result Page two 1st innings details and Page 3 2nd innings details. I can as supply the sugggested format for the text in the email once the form is sorted. Thanks
