Android devices

We need a complete reference of android devices for an app testing platform. We have composed a list of devices together with their manufacturer and need to add information about their form factor (phone or tablet) and an image for each device. The submitted file is a CSV file with one device for each row with brand and model already filled in. For each device, fill in its form factor (phone or tablet). Also find an image of the device.
