Create MS Word Template from Adobe Illustrator layout

We need a MS Word (Windows) template built, based on one that we created in Adobe Illustrator CS5 (MAC). ## Deliverables We have marked out which areas should be "locked", and which elements should be "live", and/or built into a style sheet. We can provide direction on fonts, etc. See attached sample for estimating purposes. There are 2 pages. The first would be different than the second, and all subsequent pages (which can match page 2).
