We need the reports converted to an html5 page without the use of any background images or javascript, just CSS styling. The user will use their iPad to connect to the web page and use the html pages. The html page should be responsive and built using Bootstrap v3.3.x. The styling should be implemented using CSS3. The page will contain input fields for all areas where data needs to be entered. The page look should be as close as possible to the original on the screen as well as on paper as it will be printed and must fit on a single Letter (8.5” x 11”) page. Please see attached pdf files for the two html5 pages.
