Develop Survey System with Dynamic Questions / Answers Form

In need of developing a basic survey system consisting of questions & answers as specified in DB, that will generate an (1) HTML form and capture answers and write them to DB. The answers form elements can be expressed in various ways, depending on the (2) type specified in DB / Survey Config file including (3) multiple choice / (4) single choice, (5) ranking, (6) rating, (7) comment / (8) essay box, (8) text box, (9) multiple text box etc. The form generated must be (10) responsive, meaning it will adapt to various browser widths and auto-size form elements. Upon submit it will optionally (11) save a cookie to users system, (12) record to DB the answers, (13) surveyID, (14) log IP address, (15) date/time, and (16) optionally the parameters passed via URL. There will be an option to (17) trigger a web service upon submit and (18) pass the passed url parameters . Depending on the settings, the system may (19) allow for multiple submissions from same user. Some of this URL params may be (20) shown as static text fields on the survey header. This would mean we can add such elements and link them to URL parameters. Finally, we can have A URL to call the survey can be: /doSurvey?sid=123&uid=111&strid=111&tranid=5221 Here the system will generate survey 123 for user 111. the addt parameters can be shown as static parameters. The (21) DB / Survey Config can be accessed via a URL call that will return a JSON formatted result-set with the questions / answers. To record the result set, you will (22) POST into a URL with all the answers formatted as JSON. It is important that the page (23) validate all the answers form fields according to the type and whether they are required or not.
