Create a Mobile Web form

This web is strictly corporate. This Web app is for hotels and other services. We are looking to revamp the way hotels are filing there checklist and how fast that data can be uploaded. Most service based industries use paper and pencil. I am in need of an excellent Ninja form wiz Must be extremely comfortable with ninja forms extensions. Project: Create an html5 form (prefer ninja forms) Form must have conditional logic capabilities, Multi parts and saving users progress Form must be branded and will only be used by employees and admin. Each section will require a picture upload. Individual must be efficient with email, text and Skype or google hangouts This a perfect opportunity for someone trying to break in the industry.
