Desktop application / Script that captures mobile numbers via QR code

Program / script that will allow me to create QR codes that when scanned will capture users number, then be directed to a URL of my choice. Then number is saved to file / exported to Excel SS. System also sends their text message to designated cell number I set up saying ______ scanned your code. Possible to have ID for each SMS message sent to me for tracking.
