a SPECIFIC XML form

I need “a XML form” which contains 4-5 sections: 1-Date of registration of a person 2-the name of this person 3-a small description for that person 4-a place for a small photo of that person "from his Face only" Then there will be a DIVIDER (like the one you see below…and there will be another person and so on... That means that this application has to be scrollable. The application will be implanted on of empty page of a FULL FLASH website ([login to view URL]) The purpose of this application is to register some 100-200 people You will have as much workspace for that application as an empty page on that website. I wish I had an example website to show you what I exactly need.
