Build MVP on existing Wordpress CMS

Need experienced wordpress developer who builds a minimal viable product (MVP), like 5 very easy pages, containing each an input field for e.g. for an email address and an image drop down. All pages need to be build on top of an existing responsive wordpress page. Data (e.g. email address) needs to be fetched into existing Wordpress CMS and in the same time sent by email to a me. Each activity on the page needs to be trackable, but at least, each page contains a Google analytics code. Design of all pages will be provided in .ai (Adobe CS4 Illustrator) format.
