CSS Html Sign-up Form moving

Hello We already have a form with ajax validations php and everything, you can find it here: [login to view URL] We just need to move this same form, but removing 3 fields to another page: [login to view URL] just below inside the white box that says "sign up". We would like that the new form look transparent and round like this example: [login to view URL] We need some expert in css/html/php to do this. We doesnt need a databse job or complicated ajaX validations. validations are already programmed in the first form. We just need that you move a form from one place to another, and change a little the look. Everything must be working to finish the job. Thank you
