Add a help column to Web/PHP page(repost)

The attached detailed specs and CODE and current SAMPLE screen, show our team edit web page. We need a new column added to the right of the entry fields that contains help text. For 4 fields, this help text is created dynamically based upon what the user enters on the screen.
