Application Form

I need to convert our current employment applications into a form that can be completed on our web site. The form should be in javascript and I should be able to test, tweak, and edit it using Microsoft FrontPage 2000/2002. I have attached a copy of the application in hard copy format. Upon clicking submit, the data of the form must be emailed to me with at jsteng(at)[login to view URL] [login to view URL] The ability to attach a resume would also be cool. ## Deliverables I need: A form in JaveScript that I can make changes to from time to time. The form must send me the data in the text of an email at jsteng(at)[login to view URL] I must be able to open and edit the code, if need be, in MS FP 2000 or 2002.
