Acct passwords app

I would like an app built where you have to log into the app and user creates tittle of new acct and login info for that acct. Example: I create a new user acct on [login to view URL] I then open this app and select 'new acct' then I enter login info for my [login to view URL] acct and it is stored along with a notes section. The app needs to have its own login and ability to change passwords to get into app to view other accts
