create mini geo-location (HTML5) web app

I need a small app built HTML5 geolocation web app which needs to push back the geolocation coords into a database with a flag which will then have an open ended script for me to add in some other code, it's based on the result from a QR code scan which is already made and just needs the final geo information stored, shouldn't take too long but I have no time :( Budget is around $75, anything more will be ignored, you've been warned (and don't bid and ask for more money, be up front please!!!)
