I would like a form that **takes two input fields** (username, and order#) then **outputs order status**. I'm currently using Cart Keeper shopping cart ([login to view URL]). The cart code allows a user to see the order status if he is logged in. What I need is a page [login to view URL] where a person has to enter the order# and username (email) to see the order status without having to login.
