Contact Form

I need a simple contact form. You must finish the work within 2 days. 1) I host my web on GoDaddy. 2) So, write it in php that works with GoDaddy linux hosting. 3) It must work to the order of what you see in the attached. 4) I use serifwebplusx2 wysiwyg, it must work with it. They have a 30 day trial, you'll need to download it if you are not familiar with it. I will not accept the script if I cannot use in with webplusx2. 5) Install the script for me. The attachment forms part of the requirement for this bid, please look at it carefully to know what I have in mind. Note, the boxes, field, button ALL ALREADY EXIST, your job is just to make it work. I have many forms that I want to create in the future, do this one well and I will go back with private auction in future. Thank you.
