Contact Us forms for simple PHP website

we have a pretty basic site built in PHP primarily that needs our contact us pages re-done and some new ones created. see details below ## Deliverables Contact Us Forms a. Existing Forms: Create error handling to ensure that forms get filled out correctly and add new fields i. Name (required) ii. Phone (required) iii. Email (required) iv. Time ??" Create a drop down so user has to select time (required) v. Comments (optional) ??" Create a comments area so user can enter additional information if desired. b. Thank You message ??" After Submit there needs to be a Thank You message with some friendly copy confirming that they will be contacted. c. Email notifications : i. I’m not sure how this has been setup but the email needs to be sent from a consistent (or the same) email instead of the user’s email. This will prevent emails from getting lost in spam filters. ii. Email can be formatted better so it is easier for users to read d. Data extraction ??" Is there a simple way that we can access the data? A simple table like page could be created within the Admin so that we can have easy access to the data. e. Change “get_quotes?? folder to “get-insurance-quotes?? f. New Contact Us Forms i.
