Create Access forms from exported Survey Monkey data

We have a relational database structure exported from a Survey Monkey survey. We need queries written that will pull this data together in Access forms for updates and reports after the survey is complete. We also need to be able to add new records to this data. ## Deliverables This is an urgent project, need prompt response, have already created SQL database with exported data, my problem lies with writing the queries and/or creating the views that the forms will be based on.
