development of an application using visual basic 2005 linked to Microsoft Access Database. final application will consist of an introduction page 3 instruction pages a data entry page for user. name, dob, etc linked back to database an example page test begins(timed from start to finish) 10 pages with 10 questions each in 10 different categories some questions consist of text and multiple choice using radio buttons, while other questions consist of diagrams and multiple choice using radio buttons a score will be kept throughout to give a result out of 100. some questions will have only 1 correct answer, other questions will have multiple answers that give a score on scale of 1-5. the test will be fully navigational at all times with the option to quit the test available at any stage. a menu bar is also require to show reports on previous tests and to navigate application. the application will be fully validated with appropriate messages and information boxes results will detail the score achieved in each category as well as overall score ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work [login to view URL] commented work 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
