Advanced form: error; PROFESSIONALS ONLY

I have a markup emergency people!!!!! Description: Medical release form/application that is developed in a .php file I have attached. The form converts to html and sends to an email address as an attachment. You can see the live form at [login to view URL] Problem: When a user is in Internet Explorer and they try to send the form it arrives at the email as an attachment of 0 Kb, but it works flawlessly in every other browser. Solution: This will be the third person I have had try to fix this error. If you think you are a skilled enough coder you can place a bid but I warn you, this error is in need of a real professionals eye and I need it fixed now! I will only accept a bid from a truly skilled coder. This markup error is giving me a huge headache and I need it fixed now! You must be ready to begin ASAP and you cannot just give up, if you realize you cannot do it or that you must leave you must release funds so I can find someone else to fix it, no leaving. This has to be completed NOW!
