Collect leads and offer surveys. Needs to make money - IPADCONTEST.com

I recently acquired the domain [login to view URL] and need to make money off of it. My thoughts were to give the user options of a few different surveys (from some survey providers that pay for leads). These surveys would open in new windows, passing my affiliate id or tracking id to the survey provider for tracking. each user must create an account with email address, and password. They cannot be entered into drawing until firstname, lastname, street address, city, state, zip and phone number are provided. All that information is required for shipping an ipad to the winner. I'm open to pretty much any graphic layout and any survey providers, as long as they pay (per lead/ per survey, per referral, etc)
