Create an online restaurant directory

You can view the website at www.raw-pleasure.com. We currently have a restaurant directory for raw food restaurants, but it's done in a format that's difficult to maintain. [login to view URL] We would like to switch it to a My SQL database, and be able to accept entries from people in a form over the website. Ideally we'd also like to expand it to cover a number of other areas. * Health food stores * Restaurants * Retreats * Books * Appliances Each section would have a number of fields that pertain only to it... we can provide a full design of what information is required. The online form would have a drop down as to the type of entry they were going to do, which would then give them the appropriate fields to fill in. If possible it would be good to have some sort of admin approval needed before the information was made available to the public. For people coming to our site they could search the database, in any of the categories, and provide other keywords as needed. Search etc would be placed in our standard page layout. Our website is php with OS Commerce. We have unlimited databases in our hosting plan.
