Business "Online Manual" in WordPress - needs TOC, membership passwords

I need a WordPress site developed for a business Online Manual. The Manual is used to record various procedures within the business. Think of a hard-copy instruction book with a Table of Contents, an Index, and various pages of information that can easily be updated and cross-referenced. Some of the chapters or pages can only be accessed by higher level personnel (eg managers). That's what I need in WordPress format. The must haves are: 1 Customised layout where possible (see attached idea) - not opposed to an existing theme that is customised 2 Password protection with 4 levels of access - ie level 1 can access certain posts/pages, level 2 some more, level 3 more again, and level 4 everything (administrator) 3 A numbered Table of Contents or similar. Ideally this would be a Expandable/Collapsible format (but not a deal breaker). Eg 1 Heading 1.1 Sub-head 1.1.1 sub-sub etc to about 7 levels deep (but if more, that could be a bonus) where one could just show Headers, or expand to include all etc 4 A usable search capacity or Index that is not overwhelming 5 Must be able to add pics and video easily 6 I understand the basic Word Press user interface, but any customisation needs to be easily understood and operated by updated by non-techno-geeks (some of the admin users have very basic computer skills, and will just want to paste info into a standard process) I currently have about 200 pages of content in a PHP version. I have a domain name and hosting. Ideally you would be able to transfer these to the new WordPress when the template etc has been finalised. (Note: Please don't try to convince me to opt for your hard-coded, PHP, Joomla etc solution - I've been bitten with these and definitely want WordPress because of consistent input/editing capability.)
