Customer Mailing List

I need a Mailing list with the following fuction. Sign up Form will have the following fields First and Last Name E-mail Address Street Address Phone Number When they click on submit an welcome e-mail will be sent to the individual. The Admin page will allow us to view everyone who signed up, remove, edit current people who have signed up. Also the admin will be able to enter a new newsletter and email everyone one who has been signed up. Also, the sign up form will have an unscribelist to allow people to remove them self from the list. Thank you for your time,
