A tranquil multiple page website

I need 2 basic websites done that are pretty much identical. I have a template to be used. Most of the websites are informational. On the first website there needs to be a form at the bottom of each of the pages that is routed into a autoresponder and sent back to the person for a double-opt in. After it is double-opted, the info asked for is sent to my email. I then will be putting together some newsletters that will be sent to them with the info they want. I will make the newsletters. The second website needs to have info, but will have two pages with info which needs to be double-opted again and depending on what info is provided, it is sent to one of two email addresses. Budget is between $200 and $400. Those who don't fall in these areas will not be chosen. Put the word: "YAH" in your proposal so I know you read this.
