We use Office 365 and require an online (browser rendered) form to be available for staff to complete. The form is only a handful of fields completed by the user which then gets logged. An admin distribution group should be notified of the submitted form and only an HR security group should be able to see the completed forms. I've attached the current form. The name should be auto populated by the username of the user completing the form.

You want a SharePoint sickness form to be built which would be filled up by staff and HR to access once submitted. Additionally, there would an email notifications sent out. Relevant Skills and Experience - I come with a decade of experience in designing and developing Enterprise business solutions specifically using SharePoint online, Office 365, PowerApps, Power BI, Flow, Planner and so on. Hello We can use "Infopath" form to design online sickness form. We can use "workflow" so that admin distribution group will get a notification about form submission. I understand that you want online sickness for your staff and it should select auto-fill the staff member name according to his/her username.
