Employee questionaire

I need an Employee questionaire that is a fillable form per the attached. I need our logo on the top (see attached). I have written in the changes needed but will also write out the changes to be made to this form: Add "Employee" after "All About You" Add - "Employee". Delete first three lines "Date/referred by", "phone number Add "Private before "E-mail Address". Add "Signficant Other" to read "Spouse/SIgnificant Other Name" Add "Home" to make "address" read "Home Address". Under Pets, add "Species" after "Type" in parenthesis. Add "Sports" to My/Our Favorite Team to read "My/Our Favorite Sports Team". Add at the bottom the three additional questions: "My/Our Favorite Relaxation method" , "My/Our Work Pet Peeves" and "My/Our favorite Charity"
