SBC Content layout and design

I am creating a new interactive program to be delivered both on line and in person. I have the majority of the content and need (1) template for the content - 52 pages different topics similar layout (2) Electronic forms that clients can be e-mailed or worked on (on line) to fill in and save for themselves or send back to me -saved as a document (3) graphics for each topic. An example would be an intake questionnaire or a lesson on a specific topic with follow up homework.
