Redesign of the "Cheque Payment" for my woocommerce Shop

I need you to redesign the "cheque payment" option in my shop. I want to get 2 beautiful input boxes for "paysafecard" and "amazon" code inputs instead of that basical "cheque payment" option. The customer has to see 2 input boxes where he can input either an amazon code or paysafecard code if he go to the "cheque" payment page of my shop. That additional information must be automaticly send to my shop email with other informations like order number from the order-page (cheque plugin does it already) The order my not be send to me before the customer fill out the amazon or paysafecard code into at least one of that 2 boxes!
