Life Coaching Electronic File

I am seeking an app to that would allow me to create and maintain electronic records for life coaching clients, multiple coaches. I would to create intake documents with a coaching agreement for clients to sign, use an electronic version of the Life Wheel Assessment that would link results to client file, store sessions notes and closure documents. A current client list to access client files to enter new notes A closed client list to access previous client information. A similar app is located on google suite apps . "Coaching Lobby" . I would like the app to be web-based and accessible to coaches on our devices (iPads, MacBooks) and only when the coaches domain accounts are active.
