77065 Contact Manager Script

We need a simple contact manager script, which will store contacts and allow admins/users to add comments/notes about the contact. It also need to be able to cearch for a contact/group. It needs to have an admin side where we can add/delete admins/users. Users need be able to have assigned rights as an Admin where they can add/delete records or as a "user" where they can only add records and comments. The fileds required are: Company Contact Alt. contact: Address Line 1 Address Line 2 City State (from a drop down list) Zip Country (from a drop down list) Tel Fax Email URL Alt. Contact tel Alt. Contact email Alt. Contact fax Comments Group Record Created by: Users need to log-in and when they add a record or comment it needs add who created the rcord and when they add a comment/notes it needs to show who added the comment/notes. We also need to be able to extract any of the fields to a txt file for use with an email/fax merge. We don't have a large bduget for this project, but whatever we agree on we will pay. As per all of our postings here, we are a reputable company and will only pay for work after it is complete and working on our servers. If that is not acceptable to you, please don't bother to bid.
