300626 Quick User Login/Admin

I need a quick and simple php/mysql .sql to create the database and php pages to allow users to (1)view account info, (2)make changes, (3)show logged in as user_name, (4)logout. 3 and 4 will code I will add to header of site. All I need is php code and db create file. Sorry the budget is so small but this is very limited and should take little time.
