35642 Proxy Form Script

Hi, I'm looking for someone to make me a script where I can basically just enter in an address in a form and it will proxy it on a proxy site. An example is like on [login to view URL] on the right side. I would like to have it almost exactly like that with all of those proxies in it. If you it would be cheaper I would only like a few of them and I can do the rest. I would like almost an exact copy of the [login to view URL] one though. I would also like one where I can just do one proxy at a time, so they cannot pick the proxy and do not know which proxy they are going to. So basically they just enter something into a box and the proxy will be picked by me. Please only bid if you know how to do this and you are serious. Thanks! P.S.
