Hi I have an existing web form that is operated by formdesk.com. When data is entered into the form, it's output to a database file, then the fields inserted into pdf which is then emailed to the customer. I'm looking to have this coded in php/html so that we can host it locally.

Hi, I can help you in your project creating the PHP script with the form, storing information on a MySQL DB and sending a copy with all the input in a PDF by email.
