Based on your requirements, it appears that most of the changes are in the front-end code. On the front-end, support for multiple questions can be added by re-using some of the existing code and using classes to identify each element within a row. Supporting multiple questions would also require some changes to the markup and page structure. In order to implement a survey with multiple questions, the server should be able to accept requests with multiple questions as well as return multiple results.
